Enrollment Process

Our admission process was developed to assist parents, guardians and school administrators in evaluating prospective students.  The process evaluates academic achievement, social-emotional behaviors, developmental and cognitive abilities, and disciplinary record.

For preschool enrollment, refer to information under Little Cardinal Enrollment.
 
For kindergarten enrollment, please submit the online Student Application and we will be in contact with you to move forward with the registration process.
 
Enrollment Process (grades 1 through 12, only):
  1. Create an online account and submit student information for each child applicant Student Application.
  2. Use the online account to upload the following student information:
    • final report cards from the previous two years
    • most recent report card from the current year
    • Individualized Education Plan (IEP) or Special Education Plan (EP), if applicable*
    • Chapter 15/Section 504 Plan, if applicable*
  3. After uploading all student information to the online account, complete the tuition financial aid application (below). This request is in the best interest of your family as we want to make sure financially, a private education is agreeable with your budget, prior to your child conducting shadow days or assessment tests.
  4. Student information will be reviewed by our school principals and the parents or legal guardian will be notified to schedule a student shadow day.
  5. Prospective students will attend a shadow day in which they will follow an assigned Cardinal Ambassador student in their grade level for an entire school day.  During the shadow visit, the prospective student will a.) be evaluated by teachers during class sessions, b.) complete a math/reading assessment test (administered by school principal), and c.) meet with the school guidance counselor.  Shadow days are not performed during summer breaks, however math/reading assessments and a counselor meeting is required.
  6. After the shadow visit, the principals will make a final determination on acceptance to the school at which time an official acceptance, or declined acceptance, will be issued for each prospective student.  When accepted, a written letter will be sent by the Admissions Director to the parents or legal guardian for record.
  7. The final step involves parents or legal guardians reviewing the tuition invoice and providing authorization to the school to move forward with enrollment.  DuBois Central Catholic School will then transfer official student records from the previously attended school.
*DuBois Central Catholic School requires submittal of IEP, EP, and Chapter 504 plans for the evaluation of each prospective student.  At this time, our school does not offer special service programs that can meet the educational needs and accommodations that are sometimes required in these plans.  However, the plans are evaluated on a case-by-case basis, and do not preclude student applicants from acceptance.
 
All students accepted to DuBois Central Catholic School are placed on a 30-day provisional acceptance until receipt of official records (transcripts) from the previously attended school, and the following:
  • Completed final enrollment paperwork requested by DuBois Central Catholic School.
  • Copy of student’s birth certificate or legal documentation showing a change of name for the student, if applicable.
  • Legal documentation showing custodial rights, if applicable.
  • Immunization Records
  • Completed PIAA transfer waiver forms, or other applicable PIAA forms, as required by Mr. Phil Esposito, Athletic Director, for students in grades 7 to 12 opting to join sports teams.
